March 31, 20xx, is the LAST day for sign-ups. 

We will not take late registrations for this event. NO EXCEPTIONS. 

The price of the ticket covers the cost of the venue, food, drink, bus transportation, security, prizes of all kinds, and more fun than your Senior can imagine!

Please note: Registration fees are NON-REFUNDABLE

In the event that there are any funds remaining the PFA has traditionally donated that back to the school as a gift in honor of the class.   

How to Register for Grad Night

Step Three - Fill out and Print the Leslie Bill Form - you will need to make sure this is completed in order to board your bus

The Consent and Authorization form is like your permission slip! 

It must be completed and signed by the parent/guardian AND student, EVEN IF THE STUDENT IS 18.

IN COMPLIANCE WITH CALIFORNIA STATE LAW - “AB 3628 – The Leslie Bill”

This form must be completed in order to board your bus.  All students will be checked-in prior to boarding.

Our policies regarding Alcohol and Drugs comply with the applicable laws in the state of California. We appreciate you assisting us in reviewing the following regulations with your minor son/daughter.

1.  All pick up and drop off locations must be predetermined and approved by the parent, or legal guardian executing this agreement.

Your Chauffeur will be instructed to obtain permission of a parent or legal guardian in the event of any changes to the itinerary. A contact phone number must be provided to your chauffeur on, or prior to the date of service.

2.  At all times the privacy divider inside the limousine will be open and locked. (In case of emergency, chauffeur must have access to all passengers)

3.  The following are strictly prohibited, and any violation is subject to immediate termination of our service: a.) Drinking, or carrying/storing alcoholic beverages of any type inside our vehicles are prohibited; b.) Smoking tobacco, or any other substances is prohibited inside of our vehicles;

c.)  Consuming or carrying any type of illegal drug is prohibited inside of our vehicles;

d.)  Having intimate relations in our vehicles is prohibited;

e.)  Hanging any part of the body outside the windows, or moon roof is prohibited;

f.) Vandalizing, purposely damaging, staining, purposely breaking any part of the vehicle are prohibited and subjected to additional charges for repair, estimated by professionals.

g.)  All passengers enter and exit the vehicle as a group, no splitting up the group.

NOTE: IN THE EVENT OF ANY VIOLATION OF THE ABOVE REGULATIONS, OUR CHAUFFEUR WILL IMMEDIATELY SUSPEND/TERMINATE THE SERVICE AND NOTIFY THE OFFICE TO DROP ALL PASSENGERS AT THE INITIAL PICKUP POINT, OR NEAREST POLICE STATION. NO REFUND WILL BE MADE.

PARENTAL AUTHORIZATION FORM

CALIFORNIA STATE LAW “AB 830” – SCHOOL PUPIL TRANSPORTATION

Section 12517.45 CVC...(a),(1),(C) a parental authorization form for each pupil signed by a parent, orlegal guardian of the pupil that gives permission for that pupil to be transported to or from the school or school related activity.

STEP 1: Make sure every student/pupil that is riding in the hired vehicle has this form signed by his or her parent or legal guardian. STEP 2: Students must provide this form to the chauffeur before entering a hired vehicle.

STEP 3: At least one Parent/Guardian must be present before minors boarding in vehicle/limousine. No parent/guardian present, Driver will not let any minor inside vehicle.
